If you provide written authorization to share information with a third party (e.g. physician, family member, school)

If there is a risk of serious harm to yourself or others

If we are mandated to report child abuse, elder abuse, or dependent adult abuse

2. Use of Personal Information (Website and Booking)

When you visit our website or book a session, we may collect limited personal information, such as:

  • Your name, email address, and phone number
  • Preferred contact method and session time
  • Any message or note you choose to include in the booking form

This information is used strictly to manage appointments, communicate with you, and provide appropriate care. It will never be sold, rented, or shared with third parties for marketing purposes.
